[How we work](https://arbocode.com/process)

Health systems, telehealth startups and digital health vendors all hit the same wall. Patients have to want to use the software, clinicians have to accept it inside an EHR-driven workflow, and it must never become the reason an audit fails. Each feature decision passes through one filter: whether protected health information reaches a new system, and how that system can show months later that it handled the data correctly.

We build patient portals, clinician-facing tools and backend services that exchange data with Epic, Oracle Health (formerly Cerner) and athenahealth over HL7 FHIR APIs rather than screen-scraped workarounds. Encryption, audit logging and role-based access sit in the data model from the first schema, not as a compliance pass bolted on before launch.

We sign a business associate agreement before any PHI reaches a development environment, and we scope cloud infrastructure on AWS, Azure or Google Cloud so the BAA with the hosting provider covers every service that touches patient data, including logging and backups.

The problem

## The hard parts of healthcare software

- ### PHI touches more systems than anyone tracks
Patient data moves between the EHR, a scheduling system, a patient portal, a billing clearinghouse and sometimes a wearable feed. Each hop is a place where an access log can go missing or an encryption setting can slip.
- ### EHR integration is the real bottleneck
Epic's App Orchard, Oracle Health's Ignite APIs and athenahealth's Marketplace each carry their own certification process, sandbox quirks and rate limits. A feature that works against a test FHIR server can still fail against a hospital's live instance.
- ### E-prescribing rules constrain the interface
Controlled substance prescribing has to meet DEA EPCS identity-proofing and two-factor signing requirements, which limits how much a prescribing screen can be simplified without breaking the audit trail.
- ### Clinician time is the scarcest resource
A portal or documentation tool that adds even ninety extra seconds per patient encounter gets abandoned at the bedside, regardless of how complete its feature set is on paper.
- ### Interoperability mandates keep shifting
ONC's information blocking rules and evolving USCDI data classes mean an API built to last year's requirement can fall out of compliance without a single line of your code changing.

What we build

## Healthcare systems we build

- ### Patient portals and intake
Scheduling, digital forms, secure messaging and results delivery, built against your EHR's patient-facing API instead of a bolt-on portal that duplicates records.
- ### Clinical workflow tools
Order sets, care coordination boards and clinician documentation aids that read and write to the EHR through FHIR rather than existing as a second, disconnected system of record.
- ### Telehealth platforms
Video visits, asynchronous consults and remote monitoring intake, with session encryption and consent capture that a compliance review can trace end to end.
- ### HL7 FHIR interface engines
Integration layers, using Mirth Connect, Redox or a custom FHIR service, that translate between legacy HL7 v2 feeds and modern FHIR resources without losing message-level audit history.
- ### Claims and eligibility tooling
Real-time eligibility checks and claims status lookups against clearinghouses, cutting the manual calls front-office staff make to verify a patient's coverage.
- ### Population health analytics
Dashboards that pull from the clinical data warehouse to track quality measures, readmission risk and care gaps without exposing raw PHI to the reporting layer.

## Regulations and standards we design for

- HIPAA Security Rule technical safeguards, including access controls, audit logging, integrity checks and transmission encryption, built into the architecture from the first schema.
- HITECH Act breach notification requirements, which shape how we log and retain access records so a breach investigation has a real audit trail to work from.
- 42 CFR Part 2 protections for substance use disorder records, where consent rules are stricter than standard HIPAA disclosure rules.
- DEA EPCS requirements for electronic prescribing of controlled substances, covering identity proofing and two-factor signing.
- ONC Health IT information blocking rules and USCDI data class requirements for patient-facing APIs.
- GDPR and UK GDPR for platforms serving patients in the EU and UK, layered on top of HIPAA where a platform operates in both markets.

## Platforms we integrate with

- Epic App Orchard and Epic on FHIR
- Oracle Health (Cerner) Ignite APIs
- athenahealth Marketplace APIs
- Redox and Mirth Connect interface engines
- Surescripts e-prescribing network
- Twilio for patient reminders and secure messaging
- Stripe for patient payment and co-pay collection

## HL7 FHIR integration without breaking the EHR contract

Most healthcare integration failures happen at the boundary between what a sandbox promises and what a live EHR instance actually returns. We build against FHIR R4 resources (Patient, Encounter, Observation, MedicationRequest, DocumentReference) using SMART on FHIR launch sequences and OAuth2 scopes, and we test against the vendor's certification environment before touching a production tenant.

Where a hospital still runs HL7 v2 feeds for ADT or lab results alongside a newer FHIR API, we build the interface engine to carry both, mapping v2 segments to FHIR resources instead of forcing a hospital to migrate everything at once. Rate limits, webhook retries and version drift between a health system's sandbox and production FHIR server get handled in the integration layer, not discovered after go-live.

- SMART on FHIR app launch and OAuth2 scope negotiation
- HL7 v2 to FHIR R4 resource mapping for legacy ADT and lab feeds
- Webhook-based sync with retry and dead-letter handling for dropped messages

## Designing for the HIPAA audit before it happens

We design the audit trail as a feature with its own requirements and tests. Every read and write to a record that could contain PHI gets logged immutably, with the acting user, timestamp and reason code captured at the API layer rather than reconstructed later from application logs.

Encryption key management, break-glass access for emergency record viewing, and the chain of business associate agreements across every subprocessor (hosting, email delivery, error tracking) get documented as part of delivery, so your compliance team has a working answer the first time a health plan or auditor asks for it.

## Telehealth infrastructure across state licensure lines

A telehealth platform that works in one state can violate another state's rules the moment a patient logs in from across a border. We build state-awareness into scheduling and provider matching so a visit only routes to a clinician licensed (or covered under an interstate compact) in the patient's current state, and we capture consent and session recording preferences per visit rather than once at signup.

FAQ

## Healthcare software FAQs

How long does it take to build a HIPAA-compliant telehealth platform?

A focused telehealth MVP (video visits, scheduling, intake forms) typically runs four to six months. Timeline grows with the number of EHR integrations, whether e-prescribing is in scope, and how many states' licensure rules the routing logic needs to handle at launch.

Do you need our patient data before you can start building?

No. We design the schema, screens and integration contracts against synthetic or de-identified data first. A signed business associate agreement is in place before any real PHI reaches a development, staging or logging environment.

Can we integrate with Epic if we are not a hospital system?

Yes, through Epic's App Orchard, though a health system sponsor is usually required to move an app from Epic's sandbox into production. We handle the certification submission and the FHIR resource mapping; you will need at least one Epic customer willing to sponsor the connection.

What is the practical difference between HL7 v2 and FHIR for our integration?

HL7 v2 sends pipe-delimited messages (ADT, ORU, ORM) over a point-to-point interface, common in older lab and admissions systems. FHIR exposes structured REST resources over HTTPS with OAuth2 security. Many health systems run both, so our interface engines are built to translate between them rather than assume one or the other.

Does adding e-prescribing require a separate certification?

Yes. Electronic prescribing of controlled substances requires DEA EPCS compliance, which covers identity proofing and two-factor signing, plus certification through a network like Surescripts. We scope this separately from standard e-prescribing since the identity-proofing workflow adds real development time.

Can a patient portal read appointment and lab data directly from Oracle Health (Cerner)?

Yes, through Oracle Health's Ignite APIs, which expose FHIR resources for scheduling, results and clinical summaries. Access still requires the health system to grant API credentials, and some data classes need explicit patient consent capture before the portal can display them.
