[How we work](https://arbocode.com/process)

A harder problem sits behind every patient-facing healthcare app, which is moving data correctly between systems that were never designed to talk to each other. A hospital with an HL7 v2 feed from a decades-old lab system and a modern FHIR-based EHR API needs both formats reconciled into one coherent picture. A referral from an independent practice to a specialist often shares no data model beyond a fax or a phone call.

We build the systems that sit in that gap: interoperability layers that translate between HL7 v2, FHIR, and X12 EDI eligibility transactions, prior authorization workflows that automate the payer portal and fax steps most practices still do by hand, and population health analytics that first have to resolve which records across three source systems actually belong to the same patient before any dashboard can be trusted. Clinical decision support logic gets built with a documented, versioned rule set, since a recommendation with no audit trail is a liability the moment something goes wrong.

None of this work touches the patient's phone screen directly, but it determines whether the data behind that screen, and behind every clinician's dashboard, is accurate, current, and traceable back to its source.

The problem

## How healthcare projects differ

- ### HL7 v2 and FHIR have to coexist, not replace each other
A hospital's lab and imaging systems often still speak HL7 v2, while its EHR exposes a modern FHIR API, and both feeds have to reconcile into a single patient record without one format's quirks corrupting the other's data.
- ### Prior authorization automation has to bridge payers with no API
Many payers still require prior authorization through a web portal or a fax, and automating that step means building a bridge that submits and tracks requests through interfaces that were never built for machine access.
- ### Clinical decision support needs an audit trail, not just accuracy
A rules engine that recommends a clinical action has to log exactly which data and which rule version produced that recommendation, since a wrong suggestion with no traceable reasoning is a patient-safety and liability problem at once.
- ### Population health dashboards depend on resolving duplicate identities
The same patient often exists as three different records across an EHR, a lab system, and a referral network, each with a slightly different name spelling or medical record number, and a dashboard built before that identity resolution is solved will simply be wrong.
- ### Referral tracking has no shared system across independent practices
A referral from a primary care practice to a specialist frequently has no data model connecting the two organizations' systems, so tracking whether a referral was scheduled, seen, or lost requires building the connective layer that doesn't otherwise exist.

Scope

## Features we often include

- ### HL7 v2 and FHIR interoperability layer
A translation and reconciliation layer connecting legacy HL7 v2 lab and imaging feeds with modern FHIR-based EHR APIs into one patient record.
- ### Prior authorization workflow automation
Automated submission and status tracking for prior authorization requests, bridging payer portals and fax-based workflows where no API exists.
- ### Referral management and tracking
A shared tracking layer for referrals sent between independent practices, surfacing whether a referral was scheduled, completed, or lost.
- ### Clinical decision support rules engine
Versioned, auditable clinical rules with logging that ties every recommendation back to the specific data and rule version that produced it.
- ### Master patient index and identity resolution
Matching logic that resolves duplicate patient records across source systems before population health analytics or reporting relies on the data.
- ### Population health analytics dashboards
Risk-stratification and care-gap dashboards built on top of resolved patient identity data, giving care teams a reliable view across a patient population.
- ### Eligibility verification via X12 EDI
Automated 270/271 eligibility transactions run against payer clearinghouses, reducing manual eligibility checks at the front desk.
- ### Care coordination task queues
Case manager task queues generated from referral, authorization, and care-gap data, replacing spreadsheet-based tracking of open items.

Approach

## How these projects run

We start by inventorying every system already touching patient data, the EHR, the lab interface, the referral network, and the payer connections, since the interoperability architecture depends entirely on which formats and protocols already exist rather than which ones we'd prefer to build against. That inventory shapes the data model before any dashboard or workflow screen gets designed.

The interoperability layer and identity resolution logic get built and validated against one data feed first, comparing matched records against a known sample, before extending to the full set of source systems. Prior authorization and referral tracking follow once the underlying patient identity is reliable, since building workflow automation on top of unresolved duplicate records just automates the confusion faster.

FAQ

## Frequently asked questions

How do you integrate a custom system with Epic or Oracle Health?

We connect through each vendor's FHIR-based API, typically after registering the integration through Epic's App Orchard or Oracle Health's developer program, and scope the data exchange to exactly what your organization has enabled on its own FHIR server rather than assuming full access by default.

Can you automate prior authorization requests that currently require a fax or a payer portal?

Yes, we build a bridge that submits and tracks prior authorization requests through the payer's existing portal or fax workflow, since most payers still don't expose a direct API for this. It doesn't eliminate every manual step, but it removes the re-keying and status-chasing that consume staff time today.

What is a master patient index and why does population health software need one?

A master patient index resolves which records across separate source systems, an EHR, a lab system, a referral network, actually represent the same person, usually complicated by name variations or mismatched medical record numbers. Population health dashboards built before this resolution is in place will silently double-count or miss patients.

How long does it take to build an HL7 v2 and FHIR interoperability layer?

A focused integration connecting one legacy HL7 v2 feed with one FHIR-based system typically takes two to four months, with additional data sources extending the timeline based on how consistent each source system's data actually is once you look closely at it.

Do you handle X12 EDI eligibility verification transactions?

Yes, we build automated 270/271 eligibility checks against payer clearinghouses like Availity or Change Healthcare, reducing the manual eligibility verification that otherwise happens by phone or a separate payer portal at the front desk.

How is clinical decision support logic kept safe and auditable?

Every rule in the decision support engine is versioned, and every recommendation it produces logs exactly which data and which rule version generated it, so a clinical or compliance review can trace a specific recommendation back to its source rather than trusting the system's output on faith.
